What should I expect at my Initial Mental Health appointment?During this time we will build a rapport and get to know each other. We will create a treatment plan and goals at this time, based off of your diagnosis. All intake forms must be completed before your first initial appointment.

What should I expect at my Mental Health follow up appointment?During this time we will continue to build our rapport and see how you are doing on your current medication(s) and see how you are doing with the treatment plan we starting during your initial mental health appointment.

What should I expect at my Initial Medication Assisted Treatment appointment?During this time we will build a rapport and discuss what issues you are having and come up with a diagnosis and create an individualized treatment plan for you. All intake forms must be completed before your first initial appointment.

What should I expect at my Medication Assisted Treatment Follow up?During this time we will see how you are currently doing on your medication and check your compliancy with groups/therapy.

What are groups/therapy times?Clients will be allowed to perform their groups/therapy where they currently are. I will need a monthly confirmation that you are performing your groups/therapy, if you choose to continue your therapy where you currently perform your groups/therapy. If a client does not have a place to perform their groups/therapy I have offices I can recommend, or clients can perform their therapy after their medication assisted treatment appointment with the provider.

What should I do if I have an emergency after hours?If you have an emergency after hours please call 911 or go to your local emergency department.

How will I pay for my appointment?Your payment will be due at the time of service and will be taken electronically.

How will I be seeing the provider?At this time, I am only open on Fridays 9am-6pm and Tele-Health only. During your appointment you will see the provider through a HIPPA compliant video conference call platform. Through my Electronic Medical Record you will have a client portal you will sign up and will be able to see me through the integrated HIPPA compliant platform.

Where will I perform my toxicology screens?For any clients that are required to perform a toxicology screen (bi-weekly, monthly, random) will be performed through Three Rivers located in Ironton, Ohio. I will give you the address (location) during your initial appointment.

How will I fill out my paper work (consents, medical history, current/past medication(s)?Once you have made an appointment, you will be sent a welcome e-mail that will have a link for you to enroll into your client portal where all documents will be sent to you to read and sign and submit back to us electronically.

What should I do to prepare for my initial appointment?Make sure you have completed all consents and forms required by Stephens Psychiatry & Addiction Services, PLLC prior to your initial appointment.

How do I add a new question & answer?To add a new FAQ follow these steps: 1. Manage FAQs from your site dashboard or in the Editor 2. Add a new question & answer 3. Assign your FAQ to a category 4. Save and publish. You can always come back and edit your FAQs.

Can I insert an image, video, or GIF in my FAQ?Yes. To add media follow these steps: 1. Manage FAQs from your site dashboard or in the Editor 2. Create a new FAQ or edit an existing one 3. From the answer text box click on the video, image or GIF icon 4. Add media from your library and save.

How do I edit or remove the 'Frequently Asked Questions' title?You can edit the title from the FAQ 'Settings' tab in the Editor. To remove the title from your mobile app go to the 'Site & App' tab in your Owner's app and customize.
